INSTRUCTIONS TO FOLLOW:

-Mail in your broken optical wear with your NAME / PHONE NUMBER / MAILING ADDRESS / COUPONS (if any) and we will contact you with a free estimate on the needed repair once the package has been received.

-Any repair that is $10 or more, will be sent back to the sender without charging any shipping or handling fee!

**Attention: We do not repair scratches on lenses, shape lenses or make lenses for any type of frames.
